‘I Wasn’t Able to Even Open My Eyes. That’s The Extent of the Swelling I Was’

Amanda Preisinger feels anxious about her daughter’s impending 13th birthday party. It’s not due to the typical causes related to a home packed of clamorous preteen children, but because it’s the first time she will reveal her new face to acquaintances and relatives. “Obviously I’m going to inform all guests as they arrive, ‘For your information, this is not the way I appear,’” says the 30-year-old real estate agent from Southern Florida.

Her appearance is, well, a little surprising – her face puffed up and preternaturally lifted, as though secured by industrial-grade tape. Her new – and she’s eager to emphasize, short-term – appearance is the result of six aesthetic surgeries, including an endoscopic mid-facelift, performed by a doctor in Istanbul, Turkey, last month. “My spouse became emotional when he saw me for the first time because I couldn’t even open my eyes. That’s how puffy I was,” she explains to me via video call from her home. “I had to tell him: ‘Babe, I’m fine, I’m not hurting. I just appear as if someone jumped me.’”

Growing Trend of Youthful Facial Lifts

Based on cosmetic specialists, Preisinger is among a growing number of people choosing to undergo a facelift in their 20s and 30s – significantly before a decade before typical surgeons’ typical patient age range of forty to sixty. (Though many specialists are eager to highlight the professional guideline of: “We treat heredity, not years.”) “I have individuals in their late twenties requesting facial lifts,” says London-based cosmetic surgeon Georgios Orfaniotis, a former NHS consultant who specialises in the facial area. “It’s a major procedure, and I’m a bit concerned when I observe people choosing it as a personal preference.”

Understanding the Facelift Procedure

A rhytidectomy, alternatively called a facelift, lifts the tissues in the face that start to sag as we age. “Human faces are structured a bit like onion layers,” says based in Kent facial plastic surgeon Marc Pacifico. “Dermis and adipose on the top, then a layer of supportive tissue called the SMAS. Think of the facial framework as the structural foundation of the face. And that’s what causes aging; that is what becomes lax and pulls the dermis downward with it.”

Once the preserve of moneyed older people, overuse of injectables – when excessive use of injectable gels stretches the face and leads to looseness in the skin – combined with the common adoption of weight-loss drugs, cut-price surgical travel, and the development of novel, famous-figure-promoted operative methods are driving a new kind of patient towards this invasive surgery. Statistics show an 8% increase in facial lifts over the last year in the UK; while a survey revealed that the portion of youthful candidates is growing, with 32% of facelifts now conducted for individuals aged thirty-five to fifty-five.

“People are now requesting to look like the optimal form of themselves and naturally the methods are following,” comments Orfaniotis. Currently, the operation du jour is the comprehensive facial lift, a method promoted by everyone from reality TV star Kris Jenner, to fashion designer Marc Jacobs.

Deep Plane Technique

Where a conventional facelift entails elevating the SMAS, the deep plane facelift releases the facial ligaments underneath, allowing doctors to restructure the face by repositioning the deeper tissue, and avoiding the tight, stretched look occasionally caused by older methods. “We’re not putting stress on the skin because we’re targeting deeper, to the deeper tissue,” explains prestigious London plastic surgeon Rajiv Grover, who focuses on the advanced method. “It is the underlying layers that is adjusted. The dermis follows along as a passenger.” Lifting the whole facial structure as a single unit allows for a authentic-appearing and more durable outcome. It additionally implies that the surgery is no longer being used for the primary goal of anti-ageing – or rejuvenation, as the aesthetics industry terms it – but for “beautification” and facial restructuring, too. “We’re getting numerous requests from clients aged late twenties to mid-thirties for this reason,” says Grover.

Personal Journey

Preisinger had not intended on getting a facial lift – at least not at the thirty years old. But long-term using hyaluronic acid dermal fillers in an attempt at “facial balancing” implied that by the time she reached her late twenties, she was “botched”. “I didn’t undergo this because I wanted to look youthful,” she clarifies. “I did it because the filler harmed my face. I wish I had never using it. If I didn’t have the injectable, I don’t believe I would be in this situation right now.”

Whether injectable substances ever fully dissolve has long been a point of contention in the aesthetics industry. “Research have shown that they seldom completely dissolve,” states Grover, “but they move and can block lymphatic channels. A contributing factor to what we term ‘puffy appearance’ is also the reality that in a sense, the face becomes somewhat fluid-filled because its ability to remove bodily fluid has been reduced.” Although research into the area is early-stage, cautionary statements on injectables’ potential impact on the body’s drainage have been issued, and further research initiated. An esteemed face specialist, speaking on the condition of anonymity, says he would avoid using fillers in a client because of the dangers they pose. “A lot of surgeons avoid talk about this, because the manufacturers who make injectables can be quite forceful.” He’s been told accounts, he adds, of cosmetic doctors facing legal actions after expressing worries.

Decision and Procedure

For Preisinger, once she started using injectables, she felt as if she needed to continue replenishing it – especially as it began moving to other parts of her face. She was just 28 when a local plastic surgeon in Florida suggested that a face and neck lift might be what she required to finally step off the cycle of treatments. After 24 months, she ended up choosing between a selection of suggested accommodations in the city, texted to her by the clinic assistant at her doctor’s clinic.

Originally, Preisinger had scheduled an eyelid surgery – better known as blepharoplasty – but her doctor advised her that a facial lift was the right solution for the looseness she was experiencing in her face. She arranged a forehead lift, a cheek lift and a jawline surgery. The day before her surgery, 24 hours post she’d arrived alone in the country, she decided to add a mouth enhancement. “Subsequently the surgeon stated, ‘We’re going to lift the lower eyelid puffiness.’ Then he said, ‘I think if we remove some buccal fat, it will contour your face.’ Thus I ultimately including additional surgeries,” she recalls. She spent $22,000 (sixteen thousand five hundred pounds) for the six-hour operation, including a three-day|
